Where can I find a blank form: "california schedule w-2"?

They don't seem to have it at: https://www.ftb.ca.gov/forms/search/Index.aspx

California's Schedule W-2 is for computerized tax processors only, such as TurboTax, and is not available for use by the general public. If you are mailing your Calif return in you have to include your Form W-2s.

The W-2 is a federal form. It contains a "state copy" page.  You would find the IRS Form W-2 at www.IRS.gov.
