When requesting a waiver for underpayment penalty, a supporting document needs to be attached. How do you attach such a document?

What is the format of the attached document? Can this be added to the Federal as well as tor the California filing?

You cannot attach documents in TurboTax- you will have to print and mail the abatement requests .  There is no formal format, so you can write it on a blank sheet of paper.

Underpayment of estimated tax Sec. 6654 provides for an underpayment penalty if not enough estimated taxes are paid by individuals, estates, and trusts throughout the year. Sec. 6655 applies a similar penalty for corporations that fail to pay enough taxes throughout the year. The penalty may apply even if the IRS owes the taxpayer a refund.
