If the form is blank either ignore it or ask the bank why you received it.

Do you have a blank 1099-INT in your tax file and just "Assume" your financial institution put it there?
A blank 1099-INT could show up in your tax file, transferred in empty from last year, and it's just waiting for you to put in the information from your real 2023 1099-INT form.
I'm sorry, I re-wrote the question and ended up inadvertently removing an important part.
Form 1099-INT was complete except for box 17, which is how much state tax was withheld. I ended up just entering $0, since no state income tax was withheld, and it worked.
If that worked, then great.
Sometimes imported 1099-INT forms bring in extraneous unneeded information.
IF box 17 on a 1099-INT is empty, we usually tell folks to just delete everything in boxes 15,16,17 and leave them entirely blank.
I've never had anything in those 1099-INT state software boxes in .....forever (30 years or more?).
But if there were state taxes withheld, then those boxes would be needed.
