What is a document ID and where can I find it from financial institution report?

i'm guessing because  you aren't clear as to what form your referring to.  I"m guessing it's a brokerage 1099.

somewhere on the 1st page there should clearly be a line that says document ID.  if you can't find it, call the FI.
