Of course.  All income needs to be included on your tax return.

Yes, if you made $5000 or more during the 4 months.

That's not what they asked.  They asked if they needed to file THAT W-2.   If that's their total income for the year no return is required unless you want to get back any withholding.

You need to read people's questions very carefully or you should not try to answer with the first thing that pops into your head.  And the $5000 figure you are coming up with does not conform to any tax law.
