What if i lost my 1099-misc for a lawsuit settlement

I entered the amount on both federal and CA state tax but don't have the 1099-misc form to gather the ein number. check was issued from insurance company. what can I do?

You still need to enter in the information that you have; Payer, purpose and amount. However, do not enter it in the Turbo Tax program as a 1099-MISC. Instead, enter it in as "Other Reportable income."  I have included screen shots to help you.
