What if i don't have a 1099-int or 1099-ord form? do i leave lines 2a and 2b blank on the 1040?

YES ... if you have nothing to report on any line of a return you simply leave the line blank ... don't even enter zeros.

Well you do have to report all your interest even if you do not get a 1099 INT.  You must report all your income no matter how small.  Even $1 of bank interest.
