What forms are needed and are there forms that are provided via TurboTax?

I divorced last year and I am filing as head of household. I currently split the mortgage payments with my ex-spouse. We are both listed on the 1098. What forms do I need when we file taxes separately?

You will just need any tax forms that are in your name.

You may be able to deduct your share of the mortgage interest and property taxes that are reported on the Form 1098. That is, if you have enough itemized deductions to get over your standard deduction amount. You enter the Form 1098 information in the Deduction and Credits section of TurboTax, then Your Home, then Mortgage interest, Points, Refinancing, and Insurance.

We are splitting how we claim the mortgage so I will not be able to put the same numbers as shown in the 1098. Do I need a separate form to show that I am only claiming half and my ex-spouse is claiming the other half?

So it is split. Let's say 50/50. Do I just input half the amount shown in each field? Do I need to state that I am splitting it or will that cause an error?

Enter the 1098 as though you had received one for exactly half of what is shown on the form.  That will take care of it just fine.

Do I need to attach a statement to my return of how much I paid and my ex-spouse's info? Both our names are on Form 1098. If both our names are on Form 1098, we shouldn't need a statement, correct?

No, you do not need to attach a statement.  Just claim your portion.
