What form do I need to appeal for the 1095 -A that's being charged to me and didn't give consent to be signed up for never even got info. Or a card no bills nothing ?

Being charged $1300 for something that they illegally signed me up for and signed my husband up for it and he already had insurance through his work what should I do ?

If you received a 1095-A, you must include it on your tax return. However, you can contact the Marketplace to try to resolve this issue.
