What form do I need for heath insurance?

Last year taxes was denied because I was missing a form for health insurance

The only form for health insurance reported on a federal tax return would be a Form 1095-A.

A Form 1095-A is only issued if you have insurance from one of the state Marketplace Exchanges or from healthcare.gov.

If you receive a Form 1095-B or a Form 1095-C either from your employer or from a private health care provider, those are not entered on a tax return.

Do you have insurance from the Marketplace, ie Obamacare?  Then  you should get a 1095A  to enter.  If you have Ins at work or private you should get a 1095B or 1095C.  You don't enter those.

 

Did your 2021 ever get filed?

Did you accidentally try to take the credit form 8885 when you were not allowed and the IRS removed it ???

 

What is the Health Coverage Tax Credit?

The Health Coverage Tax Credit (HCTC) is a refundable tax credit that pays 72.5% of qualified health insurance premiums for eligible individuals and their families. If you qualify for HCTC, it’s claimed on Form 8885.

To qualify for this credit, you must have received one of the following types of assistance:

  • Eligible for the Trade Adjustment Assistance (TAA) program and received a Trade Readjustment Allowance or was entitled to receive TRA except you hadn’t exhausted your unemployment insurance
  • Benefits under the Alternative Trade Adjustment Assistance (ATAA) program
  • Benefits from the Reemployment Trade Adjustment Assistance (RTAA) program
  • Pension benefit payments from the Pension Benefit Guarantee Corporation (PBGC) and be age 55 to 65
