What form do I complete for making payments to my former spouse under a Qualified Domestic Relations Oder?

I was ordered by Court to start making payments to my former spouse until the CalPERS starts direct deductions from my retirement account.  What Forms do I complete and where do I enter the payments on my Tax Return?
