What do I do with a Form 1042-S with Income Code 15 as a born US citizen?

I receive an AT&T pension, managed by Fidelity Investments.  In 2017 we spent a great deal of our time in Costa Rica.  We still maintained a US address.  We are considering residency in CR and found out we needed to show a minimum of $1000/mo deposited in a CR bank.  Through the US embassy I requested and received my Social Security check via EFT to a Costa Rica Bank.  To do so I needed to provide a CR PO Box.   Somehow through SSA, AT&T was given my CR PO Box, which provided it on to Fidelity.   For two months I had a 30% tax withheld from by pension check as an NRA.  I called and confirmed my US address with Fidelity and at their request provided a W9 with the same address they had all along.   When I received my Tax documents for 1017 Fidelity gave me a 1099-R for 10 months and a 1042-S for 2 months, which shows the massive 30% deduction in box 7A.   I was born, and worked my entire live in the US.  According to the IRS codes I read, I could NEVER be a NON-Resident-Alien.

You can enter your 1042-S related to your pension distribution and your related withholdings under the 1099-R section in TurboTax.    You need to check the box for the option that "I need to prepare a substitute 1099-R" on the screen Tell Us Which 1099-R You Have.  As it is a substitute1099-R (Form 4852), you will need to provide an explanation and mail your return along with your filing copy of the 1042-S,

You will find the 1099-R under Wages & Income, scrolling down to the section IRA, 401(k), Pension Plan Withdrawals (1099-R).
