What do I do if my disclosure consent form won't accept my name even though everything matches?

I'm trying to sign my forms and it says to sign to match what is under where I'm signing and it does match, what do I do?

Double-check the My Info section. Ensure your name does not have spaces before or after it. When you are signing, make sure there are no spaces there as well.
