W2 shows Box 15 filled in as FL and Box 16 which is the same as Box 1

I live in FL where there is no state tax, and my previous Job always had it blank, so I never filled it out when filing. When I file this time, do I still leave the Box 15 and 16 blank or do I input what the W2 says? Also, would I need to have my W2 amended to have it show up as blank or could I still file and not fill in the boxes?

As your state, Florida has no state income tax, it isn't in the drop down menu of Box 15 of form W-2. Just leave boxes 15 and 16 blank even if FL is listed in box 15 and box 16 is filled with an amount on your form W-2. 

 

You can e-file with boxes 15 and 16 of your W-2 lank.
