W2 missing box 12 Codes D and AA for Simple 401(k) and Roth 401(k) respectively

For my W2 for 2021 there seems to be missing data for box 12 Codes D and AA for Simple 401(k) and Roth 401(k) respectively.  Should my employer fix this and issue me a new W2? I assume they also have to refile with the government too right?  Or is there another way to fix this?

I would suspect your employer would have to rectify this.
