W2 Box 14: What are the FSQ and SDI codes?

I'm in Massachusetts. I'm curious about employer codes for two entries in Box 14. The first says "FSQ1 511" and the other says "SDI/Other."

 

Ideas what they mean?

 State Disability Insurance is SDI. The other one would be to ask your employer. Box 14 is informational for most states.  See Instructions for Forms W-2 and W-3 (Print Version PDF) box 14 instructions. A wild guess for the 511 would be health related since 511 is the code for Health and Safety in Mass.
