TurboTax notes "We see (spouse) had a break in HDHP health plan coverage during 2018."
We didn't have a break. I had 12 months of and HDHP and it's indicated as "family". I can't figure out where/how TurboTax is thinking she had a break. And if that's an error (on the 5329 reporting).

Review the HSA entry section again carefully ... sounds like an issue with who the HSA belonged to. You may want to delete that section and complete it again.
if you have not switched to the FORMS mode try looking there ... in the left column there will be a red exclamation mark next to the form with issues ... click on that to bring it to the window ... then look for any pink boxes ... those are what need to be fixed. You may see the issue that way clearer and you may be able to make the corrections right there as long as you do not use the override function ... overriding will void the accuracy guarantee and can stop you from efiling.
This message is a flaw in the user interface. What happened is that you had an HSA and so in the HSA interview, you indicated that you had HDHP coverage. Since your spouse did not have an HSA, your spouse was never asked if he/she had HDHP coverage.
When at the end of the HSA interview, TurboTax asked what kind of HDHP coverage your spouse had on December 1, 2017, you probably answered "Family", because that was literally true. However, since your spouse did not indicate any coverage in 2018 (because he/she was never asked, not having an HSA), TurboTax incorrectly assumed that your spouse had coverage in 2017 but didn't in 2018 - hence, the "break".
In this case, when you see the question "What type of HDHP coverage did [your spouse] have on December 1, 2017?", answer "None". This is OK, because this question doesn't apply to your spouse anyway.
