The rollover was from a 401(k) retirement plan so the amount rolled over is entered on Line 4c with the word ROLLOVER on the Line for 4d.

Yes. A 401(k) per IRS instructions goes on line 4c & 4d.
"Annuities and pensions" is a catch-all for everything that is not an IRA - refer the the 1040 instructions for what goes on lines 4c &4d.

to clarify, I’m looking at my completed 1090 now and I see my husband’s rollover in box 4c and nothing in 4d. That’s correct? It initially worried me because I saw this amount right under IRA distributions.

Correct. 4c & 4D is for everything that is NOT an IRA. A rollover is not taxable so 4d should be zero of blank with the word ROLLOVER next to it.
