There is no State ID number in box 15 on W2 form. Also I have a 1099-R form do I need to do something with it?

Contact your state tax office (or Turbotax) to get state ID Number. If you have a 1099-R form, you will need to enter into fed tax form as income because it is income from a Pension, Annuity, Retirement or Profit-sharing Plan, IRA, Insurance, etc.
