Yes, this can happen if you enter your health insurance premiums in more than one place. Depending on your situation, you may be asked for it more than once.

The program will automatically carry health insurance premiums to Schedule A that have been entered as:

  • Health insurance you paid pretax out of your paycheck
  • Self-employed health insurance
  • An Affordable Care Act plan (also known as Obamacare)
  • Health insurance if you have a Form 1095-A

To remove the duplicated entry:

  1. Open your return.
  2. Select Federal, then Deductions & Credits.
  3. Scroll down to Medical Expenses, expand the section.
  4. Select Edit to the right of Medical.
  5. Select Continue until you get to the screen titled, How much did you spend on insurance premiums?
  6. Carefully review the instructions for this screen. If any of the situations listed apply to you, delete your entry from the first box.
  7. Select Continue.
  8. Review your return after this change to make sure the entry for health insurance is correct.

thanks for the advise. However per turbo tax it is a software issue that developers are working on,

problem was solved by deleting the 1099R with the duplicate insurance entry and then re entering it by hand.
