The 1099div received does not have the Canadian Co. address info to fill in the tax form, Do I use their US representatives Address info.

Yes, you can use the address of their U.S. representative. 

The IRS doesn't use addresses to identify 1099 payers. When necessary, they use FEINs and may use names if performing manual processes..
