The 1095-C is a form from your employer describing your employer coverage. This employer health coverage could be an HDHP (High Deductible Health Plan). Your HR department will know for sure.

If you have an HDHP, then you are likely to have an HSA (Health Savings Account) which is an account through which you can pay the HDHP's deductible with tax-free dollars.

If you have any contributions to or distributions from an HSA, the IRS requires you to file form 8889.

If you have a code "W" in box 12 on your W-2, then you have contributions to your HSA. If you received a 1099-SA, then you had distributions from your HDSA, both of which require the form 8889.

Form 8889 is part of TurboTax Deluxe and higher versions.
