Surrendered Life insurance policy with a portion of proceeds withheld for Federal Tax. Turbo Tax makes no provision for entering withheld amount. Misc. Income page

You should have received a 1099-R for this?

You're right, they sent one but I never received it.  Resolved with a phone call.  Thanks for responding
