Should I select I was required to repay unemployment benefits? I have a pending appeal for being overpaid

I think it was a mistake but my unemployment benefits went from approved to denied and they said I was overpaid, so I appealed it because there's no reason I was denied and the case is pending

If you did not actually make a repayment in 2021, answer NO.

If your appeal is pending and you have not repaid any unemployment benefits, then you should not say that you are required to repay unemployment benefits. You only say so if and when you actually repay.
