Received a W-2 from Class Action Settlement From Another State For Job Held 15 Years Ago as Non-Res

Received a class action settlement for withholding payments from a job I held almost 15 years ago in a state that I was not a resident (non-residential college student). Total check amount was just under $600, W-2 is for $78 and they withheld State, Medicare, and UI taxes (all totaling under $5.00). Am I required to file taxes in that state? Do I report the total settlement on my federal taxes?

You would need to report the entire settlement on your taxes. You would enter the W-2 as normal and the additional monies as "lawsuit proceeds" as described in the FAQ below (just enter the difference between the W-2 and $600 as amount received without a 1099-MISC form):

Regarding the state, it would depend on which state but I would file the return anyway to get back those taxes withheld.
