Box 4 on a 1099-R is different that Box 4 on a W-2.

Please make sure you are on the correct screen.

If the form you have is showing federal income taxes withheld in box 4 that is Not a W-2,  It is a Form 1099.

A W-2 will have federal income taxes withheld in box 2 and Social Security taxes withheld in box 4

IRS Form W-2 - https://www.irs.gov/pub/irs-pdf/fw2.pdf

If you received a Form 1099-R you do not enter it in the section for a W-2

To enter, edit or delete a form 1099-R -

Click on Federal Taxes (Personal using Home and Business)
Click on Wages and Income (Personal Income using Home and Business)
Click on I'll choose what I work on (if shown)
Scroll down to Retirement Plans and Social Security
On IRA, 401(k), Pension Plan Withdrawals (1099-R), click the start or update button

Or enter 1099-r in the Search box located in the upper right of the program screen. Click on Jump to 1099-R
