On the irs consent form the software is not allowing me to to enter the correct dates inits fromat

Refreshing your browser and clearing out your cache and cookies may resolve the issue, the articles below will help.   After performing these steps log out of TurboTax and then back in.  If the issue still persists please try a different browser.

I have tried clearing my cache on multiple browsers and multiple computers and it did not work.  The issue still persists.
