On the form 3520 after signature it asks for title. I got the money from my father as a gift, so do I say Daughter or Dr (I have a PhD) or something else?

It is a little confusing. I am I correct that there is no place to put any of his information?

You don't need to put anything for title. That would be used by a corporate filer who had a title in the company.
