On my w2 I have box 12a D and a box 12b DD which do I put on the tax form?

Both of them.  Code D is your 401(k) contribution, and DD is your health insurance covered by the company.

Both if they have amounts.   After you enter the enter the first box 12 code and amount you can add another row.
