On my W2 form I have in box 12: D and D 15. There is only a D option to select though should I add the D and D 15 values together and enter as one?

Ignore the 15 ... that just references the tax year the contribution was for  so you can either add both amounts together under one code D  or make 2 code D entries ... your choice
