The bonus should have been included in her W-2. She should contact her employer and request a corrected W-2.
If the employer does not correct the W-2 then use this procedure -
Click on Federal Taxes (Personal using Home and Business)
Click on Wages and Income (Personal Income using Home and Business)
Click on I'll choose what I work on (if shown)
Scroll down to Less Common Income
On Miscellaneous Income, 1099-A, 1099-C, click the start or update button
On the following screen - On Other income not already reported on a Form W-2 or Form 1099 click the start or update button
On Other Wages Received click on Yes
Continue and on the screen Any Other Earned Income click on Yes
On the screen Enter Source of Other Earned Income select Employee compensation not reported on a W-2 and continue
Continue through the screens and on the screen Report Your Wage Income enter the information requested and for the Reason Code select Code H and continue.
