My state requires taxes be paid on unemployment. I only

If you have no withholding in box 11 leave boxes 10a, 10b & 11 blank. 

I have only 4 boxes. Total. CA requires taxes b paid ?????

California does not tax unemployment benefits. If you don't have an entry in box 10(a) in your form 1099-G, then no tax was withheld from your unemployment benefits. If they are taxable on your state tax return, that tax will be added to the tax due on your state income tax return.
