Is entering a 1099-R required? I forgot to enter it.

Yes, it is. You must enter any and all 1099-Rs that you get. The IRS gets copies of 1099-Rs and they do check to see if you report them all. To avoid a nasty letter from the IRS, you should prepare an amended return reporting the 1099-R and then print it out and mail it to the IRS
