Code V in box 12 of your W-2 is "Income from the exercise of nonstatutory stock option(s)."
I do not know what question you are referring to (is it on your state return?), but your insuance premiums are listed with code DD in box 12 of your W-2.
NOTE: these premiums are being paid with tax-advantaged dollars, so you do not normally enter them separately on your state return. You normally are asked only about health insurance premiums that you paid with post-tax dollars.
Ah, I found the question you are probably referring to on the Iowa interview ("Tell us about your health insurance premiums"). As you can see from the red text at the bottom, you are not to enter your code DD amount. Enter only health insurance premiums that you paid for with after-tax dollars (many taxpayers do not pay for these premiums with after-tax dollars).
