Inherited 1099-R: Where is your distribution from?

My husband received $ from a life insurance policy or annuity (not entirely sure what it was) from an Aunt that passed away.  He received a 1099-R for the total distribution and box 7 has 4D.   What should we answer for the question "Where is your distribution from"?  We are in NY if it matters.

Since it's not a government-related pension, choose 'None of the above'.  A later question may ask 'was this a 403b or other qualified plan', choose 'other qualified plan'.  TurboTax may also ask about RMD.  You can enter $0 or the total amount, even if RMD is not required for this type of distribution.  

 

When entering the info from the 1099-R, in Box 7 enter 4 on the first line, and D on the second line.
