In 2017 state refund section, should "total of all your payments and withholding" include only state tax paid, or should it include SS, fed tax, Medicare, etc? W-2 box???

Have 2016 W-2, but can't figure out what "total" this question is asking... is there a line or box on the 2016 W-2 which answers this question or do I add up several boxes/lines?

There is a line in your W2 for 2016 that answers this question. 

"Total of all payments and withholdings" is the total shown in box 17, plus any additional estimated payments you may have done to the state (in addition to what is showing on box 17). 

You do not have to include social security and Medicare because those are federal numbers.

I did receive state refund (MD) for 2016. I had 4 W2's for 2016, added column 17 of all 4 W2's  (All were for MD state) and also checked itemized deductions. I am guessing the state refund to be taxable income but the state refund is not shown as income on line 10. The adjusted gross income (line 37) has no impact of state refunds. Am I doing something wrong?

Are you sure it's not box 16???? box 16 says "CO Income Tax Withheld from W-2s and 1099s, you must submit the W-2s", but box 17 has 0 in it and says "Prior-year Estimated Tax Carryforward." Seems like box 16 is the one, not 17. please reply!!
