If I receive a payment as part of a deferred compensation plan from a prior employer where do I log the income on the form. I do not see the form as a 1099 or W2

To clarify I did not receive any 1099 or a W2. It was just a statement with the check and taxes deducted

You should be getting the 1099R. They have until Jan 31 to mail it. Maybe mid February.
