I wasnt getting federal taking out all year and now im trying to set it up to prevent further penalties. I need help to figure out what to put in each box. 1st box says, "Number of qualified depende

What form are you trying to fill out?  Are you using an old version of a W-4?

And....if you have not had any tax withheld all year and expect to owe at least $1000 at tax time you will be subject to penalties.  You might want to make an estimated payment directly on  the IRS site.
