I was disabled all of 2016, and got long term disability W2 from Aetna Insurance, as a 3rd party sick pay..where do I put these wages, and federal income tax withheld?

It gets entered in the same place you would enter any regular W-2. Pay attention to the code(s) and amount(s) in box 12.
