I've entered a1099-R into TurboTax, have check box 2B as Taxable Amount Not Determined, Box seven has a distribution code of 7 and the IRA/SEP.SIMPLE box is checked. I don't see a follow-up question for QCD. ??

Check your birthdate.  You might want to temporarily change it so you are 72 but you do need to be 70 1/2 to use the QCD.  Go to the 1099R screens and continue through your 1099R to the "Uncommon Situations" screen.  You should see the question about distribution directly to charity.  After you are finished, change your age back to your actual birthday in the "My Info" screen.  The age question is related to the 72 age change for RMD.
