I used TurboTax for Fed and State but the Tax Form has "Self-Prepared" printed on it. Did I need to pay the extra $60 to say that it was prepared by TurboTax?

Actually, no. TurboTax is a tax preparation software, that guides you to prepare your own income tax return.
