I Used Form 1040EZ for HSA in 2015?

I would assume that this issue is based on buying your software rather than a change in the law.

If you have contributed to or taken money from a HSA, you must file Form 8889 with your Form 1040. If you were required to file Form 8889 then the Turbotax software forces you to file Form 1040. If you used Form 1040EZ then I suspect you did not complete the HSA interview and Form 8889 was omitted. This will likely be a problem for you.

2015 TurboTax and 2016 TurboTax indeed require the filing of Form 1040 (with Form 8889) if you or your wife made contributions to an HSA for 2015, either through an employer or as a personal deductible contribution.  If TurboTax prepared 2015 Form 1040EZ, it means that the HSA contribution was not properly entered.  Perhaps a code W amount on your wife's 2015 Form W-2 was overlooked and not entered into 2015 TurboTax.  If the contribution was a personal deductible contribution rather than through an employer, Form 1040 is required by the IRS to be able to claim the deduction (2015 Form 1040 line 25).
