Yes you do, 1040-EZ is simply the type of form used.
You can locate your AGI on the IRS website with this link.
https://www.irs.gov/individuals/get-transcript
It will allow you to view, print, or download your transcript that will have your AGI in it.
You will look on Line 4 since you filed a Form 1040EZ
