I received std payment from aflac after i had my daughter. Do i claim this on my taxes? i did not receive a tax form from aflac. this is a benefit that i pay for out of my pay check. my employer doesn't contribute towards my policy with aflac.

No, this is considered short term disability insurance. Because you paid for the policy it is not taxable and does not need to be reported.
