I received Short Term Disability payments, but didn't receive a tax form. Do I report that as income, and how if I do?

It depends. If you paid for short term disability out of your paycheck, it would not be taxable to you. If your employer paid, it would be taxable to you. You may have each paid part. See Are Short-Term Disability Claim Payments Considered Earnings.
