If the dividends you received were taxable to you, you should have received a 1099-R. If what you received was the pay out of an insurance policy to you as the beneficiary, then it is not taxable. But it's hard to say for sure, without more information.
This was dividends from MetLife policy my dad had before he passed away it was not taxed to me nor was I the beneficiary of this .I had to apply for the unclaimed funds from the state of New York because no one else was doing it in my family and didn't want to deal with it...so I did it...and then I got a 1099 – B form in the mail And two boxes are checked on the form number five and number six and number five says if this box is checked it is non-covered security and box number six says reported to IRS gross proceeds..
Is the 1099-B in your name or your dad's name? If it's your name, you will have to report it on your tax return.

File an amended return. If you do nothing, the IRS will eventually send you a letter with a proposed adjustment to your return. You could wait for that, but the clock is ticking on penalty and interest on the additional tax. The letter could arrive in a matter of weeks, or it could be more than a year.
