I received fitness reimbursement from my health insurance directly in form of a cheque. But no 1099 form ? So, is it taxable ?

I am asking this because my health insurance is through my employer.
So, If "YES" it is taxable then how should I report it in turbo tax ?

No, the reimbursement is a refund of your health insurance payments and it is not taxable.

But then why they said in the reimbursement form " Disclaimer : That this might be taxable income "  ?
 Also my employer charges me for Health insurance with pre-tax dollars . So, is the reimbursement still not taxable ?
