I received a severance payout from bankruptcy administrators. They withheld tax, but didn't provide anything like a W2 or 1099...where does this get recorded?

The check came from the administration company, not my former employer...do I manually add it to the items from my W2?  Or do I need the tax ID from the administration company?

If fed/state/FICA taxes were withheld then then they  MUST issue a W-2 form ... contact the issuer for the missing form.
