I received a 1099-MISC from a business I do not own. What do I put as the address, EIN, etc.?

I understand that the IRS considers nonemployee compensation the same as self-employment earnings but I am still confused as to what I should be filling out in Turbotax. 

The business is based in Washington. I live in a different state.  What do I put as the business address? I "work" from home, should I put my home address?

The "job" involves doing simple work like surveys, giving feedback on advertisements, etc. basically small tasks that do not involve more than around 10 minutes of effort a day and it's more something I do in my free time, but still on a daily basis. It's not my main source of income and I do not even consider it a job. Would this still be considered "actively participating in this work" though?

My 1099-misc I received also has the business' TIN. Do I enter this as the EIN on Turbotax?

Enter your address as the address of the 1099-MISC.  Your SSN takes place of the EIN in this case.

It would still be considered "actively participating.
