I received $4,500 as a stipend.. Should I receive a 1099 (or other form to file with my taxes) if I completed a W-9 for an organization?

Yes, the organization should send you a Form 1099-MISC for that income.  If you prefer not to wait on the tax form, you can go ahead and report the income now under Federal Taxes>Wages & Income>Less Common Income>Miscellaneous Income>Other Reportable Income.
